Eligiblity

Criteria

To be eligible for admission to the hospice program here are some general criteria:

• The patient has been diagnosed with a terminal illness or health decline with a life expectancy of six months or less if the disease progresses as expected. Some illnesses related to hospice patients are:
  • A.L.S. (Lou Gehrig's Disease)
  • Cancer
  • Chronic Degenerative Neurological Disease
  • Dementia
  • General Debility
  • Heart Disease
  • HIV/AIDS
  • Huntington Disease
  • Liver Disease
  • Parkinson's Disease
  • Pulmonary Disease
  • Renal Failure
  • Stroke or Coma
• The attending physician has agreed that the patient is a suitable candidate for hospice care.
• The patient/family desire hospice care and are no longer seeking aggressive or curative treatment.
